The Secretary must include notice of this subchapter's rules on the hospital-care application form. This applies to the form for hospital treatment and domiciliary care.
The Secretary* must create an application form for hospital treatment and domiciliary care. That form must include notice of the rules in this subchapter.
The Secretary* shall prescribe a form of application for hospital treatment and domiciliary care which shall include notice of the provisions of this subchapter.
